As a cancer care coordinator, I will be working across 7 Surgeries within the Rural Rother area. The primary objective being to work closely with each Surgery and other Primary Care professionals within the PCN to identify caseloads of patients ensuring that the appropriate support is made available to them and their families. Working within the DES guidelines for Early Cancer Diagnosis.
