I am a Care Coordinator Tackling Health Inequalities working as part of the Rural Rother PCN, I joined the team in September 2022.

My job involves working with the 7 Primary Care Network Practices where I build up profiles to tackle health inequalities in Rural Rother. I work with clinical directors, GP clinical lead, Business manager, Practice managers and PCN ARRS team and community stakeholder to support patients with the health inequalities cohort to access medical and social care
